What is Urachal Adenocarcinoma of the Ureter?

Urachal adenocarcinoma of the ureter is a very rare type of cancer that develops in the urachus, a structure present during fetal development that connects the belly button to the bladder. In most cases, the urachus closes up before birth and becomes a small ligament, but in some cases, it may persist and become a site for cancer development. Urachal adenocarcinomas are most commonly found in the bladder, but on rare occasions, they can be associated with the ureter.

Symptoms

Symptoms of urachal adenocarcinoma of the ureter may include:

  • Blood in the urine (hematuria)
  • Pain during urination
  • Urinary tract infections
  • Abdominal pain or discomfort
  • A palpable mass in the lower abdomen

Disease Aetiology (Causes)

The exact cause of urachal adenocarcinoma of the ureter is not known, given the rarity of the disease. It is believed to originate from glandular cells in a persistent urachus or urachal cyst.

Diagnosis

The diagnosis of urachal adenocarcinoma often involves:

  • Detailed medical history and physical examination
  • Urine tests to check for blood or signs of infection
  • Imaging tests such as ultrasound, CT scan, or MRI to identify the presence and extent of the tumour
  • Biopsy of the tumour for histopathological examination

Treatment

The treatment for urachal adenocarcinoma of the ureter typically involves:

  • Surgery: The primary treatment, often involves wide local excision of the tumour along with a portion of the bladder and the umbilicus (belly button).
  • Chemotherapy: May be used after surgery to kill any remaining cancer cells and reduce the risk of recurrence.
