What is Malignant Mixed Mullerian Tumour of the Vagina?

A malignant mixed Mullerian tumour (MMMT) of the vagina, also known as carcinosarcoma, is an extremely rare and aggressive form of cancer that contains both carcinomatous (cancer of the cells lining the vagina) and sarcomatous (cancer of the connective tissue of the vagina) components. This cancer primarily occurs in postmenopausal women, and due to its rarity, our understanding of its aetiology, optimal treatment strategies, and prognosis is limited.

Disease Aetiology (Causes)

The exact cause of malignant mixed Mullerian tumour is unknown, but it likely involves genetic mutations that lead to uncontrolled growth and division of the cells, resulting in a tumour. Risk factors may include age, exposure to certain forms of the hormone estrogen, and history of pelvic radiation therapy.

Diagnosis

Diagnosis of malignant mixed Mullerian tumour often involves:

  • Detailed medical history and physical examination
  • Biopsy of suspicious tissues
  • Imaging tests such as ultrasound, CT scan, or MRI to ascertain the size, location, and extent of the tumour

Treatment

The treatment for malignant mixed Mullerian tumour of the vagina typically includes a combination of strategies:

Surgery: This is often the primary treatment, aiming to remove the tumour and any nearby lymph nodes that may be affected.

Radiation Therapy: This can be used before surgery to shrink the tumour, or after surgery to kill any remaining cancer cells.

Chemotherapy: This can be used in combination with surgery and radiation therapy, particularly in more advanced cases or if the cancer recurs.
