What Is Carcinosarcoma of the Lung?

Carcinosarcoma of the lung is a rare and aggressive type of cancer that features both carcinomatous (epithelial) and sarcomatous (mesenchymal) components. This dual nature makes it a complex and challenging condition to treat. Carcinosarcomas account for a very small fraction of lung tumours and are known for their rapid growth and high potential for metastasis.

Symptoms

The symptoms of carcinosarcoma of the lung are similar to other lung cancers and may include:

  • Persistent cough, which may be dry or produce phlegm
  • Coughing up blood or bloody phlegm
  • Shortness of breath
  • Chest pain or discomfort
  • Unexplained weight loss
  • Fatigue
  • Recurrent respiratory infections such as pneumonia or bronchitis

Disease Aetiology (Causes)

The exact cause of carcinosarcoma of the lung is not fully understood. Risk factors are similar to other types of lung cancer, including:

  • Tobacco smoking, which is the most significant risk factor
  • Exposure to asbestos, radon gas, and other carcinogens
  • A history of lung diseases or lung infections
  • Genetic predisposition

Diagnosis

Diagnosing carcinosarcoma of the lung involves a series of steps:

  • Imaging tests, such as chest X-rays, computed tomography (CT) scans, and magnetic resonance imaging (MRI), to identify the presence and extent of the tumour.
  • Bronchoscopy or thoracoscopy to visually inspect the lungs and collect tissue samples.
  • Biopsy of the tumour, with histopathological examination crucial for identifying the biphasic nature of carcinosarcoma.

Treatment

Treatment for carcinosarcoma of the lung typically involves a multimodal approach, including:

  • Surgery to remove the tumour is often the first line of treatment, provided the cancer has not extensively metastasized.
  • Chemotherapy may be used before surgery (neoadjuvant chemotherapy) to shrink the tumour or after surgery (adjuvant chemotherapy) to target any remaining cancer cells.
  • Radiation therapy is another option, either as a primary treatment when surgery is not possible or as an adjunct to surgery and chemotherapy.
  • Targeted therapy and immunotherapy are being explored in clinical trials and may offer new treatment avenues based on the tumour’s specific genetic markers.
