What is Nodular Melanoma?
Nodular melanoma is a highly aggressive form of skin cancer that originates in the pigment-producing cells (melanocytes) of the skin. It is known for its rapid growth and deep invasion of skin layers. Unlike other types of melanoma, nodular melanoma typically grows more vertically (downward) than horizontally and it doesn’t always follow the usual melanoma signs (ABCDE rule). It can appear anywhere on the body but is most common on the trunk, head, and neck.
Symptoms
The primary symptom of nodular melanoma is a firm bump on the skin that is usually black, but can also be brown, tan, red, blue, grey, white, or skin-toned. Other signs and symptoms can include:
- A new growth on the skin
- A spot or growth that feels different from the surrounding skin
- Rapid growth over a period of weeks to months
- Itching or bleeding from the lesion
- Pain or tenderness in the lesion
Disease Aetiology (Causes)
The exact cause of nodular melanoma is unclear, but it’s thought to involve damage to the DNA of skin cells, often due to ultraviolet radiation from sun exposure or tanning beds. Some individuals may also have a genetic predisposition to this type of cancer.
Diagnosis
Diagnosing nodular melanoma typically involves:
- A thorough skin examination
- Dermoscopy, a technique that uses a special magnifying lens to examine the skin
- Skin biopsy, which involves removing a small sample of the suspicious skin lesion for laboratory testing
- Further imaging tests like CT, MRI or PET scans if the cancer is suspected to have spread
Treatment
Treatment options for nodular melanoma include:
- Surgery to remove the melanoma and a margin of normal skin around it
- Sentinel lymph node biopsy to check if the cancer has spread to the nearest lymph nodes
- Adjuvant therapy, such as immunotherapy or targeted therapy, may be recommended after surgery if there’s a high risk of recurrence
- Chemotherapy, immunotherapy, or targeted therapy for advanced or metastatic cases
